"Suddenly, Karen collapses and Lucas warns Dan that he will shoot him if he goes near Karen. As Dan motions towards Karen, Lucas pulls the trigger, but misses Dan on purpose. He sends Dan on his way and takes Karen to hospital, where she is diagnosed with eclampsia and Lucas is told that she will need an emergency C-section; a baby girl is born though Karen's life is still endangered. When Deb and Nathan learn that Dan killed Keith, Nathan disowns his father. After visiting his niece in the hospital, Dan turns himself into the police while Karen flatlines in the hospital and Haley goes into labor in the middle of her valedictorian speech.
Haley has a son, James Lucas Scott, and Karen has a daughter, Lily Roe Scott; while in the hospital, she has a dream in which she introduces Keith to his daughter). Dan is jailed and tries hanging himself in his cell, but fails. Brooke has sex with Chase, but is still going away to California with Peyton. Peyton doesn't want to leave Lucas, but he insists, saying she is destined for greatness. Rachel shows up after her absence and throws a party for the graduating --and incoming--seniors. Brooke gives Rachel her diploma. Mouth musters up the courage and kisses Brooke ("I've always wanted to do that!" he says) and also makes peace with Shelley. While at the party, Nathan and Haley leave James in the care of Deb, and get nervous because of her history (Nathan calls to check up on the six times, while Haley calls eight times). Both are fine, and Deb really seems to have her problems behind her. Lucas and Nathan consider visiting Dan in his cell (as Karen did, saying she will one day have to explain that Lily's daddy was killed by the little brother he loved so much), but they ultimately decide that they will never visit him. Brooke and Lucas are named the godparents to James, and Lucas reveals that he will be Whitey's assistant coach at Nathan's college. The season ends much as the first season began: with a pick-up basketball game on the river court, this time guys vs. girls."

Hilarie Burton confirmed on TRL that the show's fifth season will focus on their lives after college, citing the reason for this as "We've seen the High School dramas not do so well while they're in college, and another thing, we've done everything that everyone does in college". She said this will "enable them to do flashbacks and a lot of cool stuff with the characters". The fifth season now has a 22-episode order"
